Choose the correct version of the order form based on the date that you would like your materials to arrive in the schools. If the wrong order form is used, there will be delivery problems. If you are ordering materials to arrive in schools from April 1, 2007 to June 30, 2007, only use these forms:

• Chapter Order Form (“Wings and Things” 2006/07 edition)
• Teacher Enrollment Order Form (“Wings and Things” 2006/07 edition)

If you are ordering for materials to arrive in schools from September 1, 2007 to June 30, 2008, ONLY use these forms:

• Chapter Order Form (“Back From the Brink” 2007/08 edition)
• Teacher Enrollment Order Form (“Back From the Brink” 2007/08 edition)
